Near miss/Accident example – 1

Brief Description near miss/accident:

Pile boring works were under progress by Hydraulic Rotary Drilling Rig and during this process, a fault was detected in Radiator Suction Hose Pipe by Operator. The faulty Hosepipe was replaced & Mechanic checked Rig Machine for running & positioned the Kelley in the vertical position. Thereafter the operator switched off the Rig Machine & left the site due to change in shift duty

During the site visit the area by night shift supervisors, the fire was noticed in Rig Machine. Site Staff immediately tried to extinguish the fire by using the fire extinguishers but they were unable to control the fire & immediately intimated to Fire & Safety Dept. Fire & Safety rushed to the incident spot and controlled the fire.

PROBABLE CAUSE OF THE INCIDENT:

Since Pile boring works by Hydraulic Rig was under progress in close proximity of running flare and after closing the day shift activity, Rig Machine was switched off and left by the operator in unmanned position at the same working location. Therefore, Fireball from Flare vertical riser might have fallen on the floor of Hydraulic Rig Machine, which must have been wet with diesel, a flammable substance, which has resulted in the fire inside the Hydraulic Rig Machine.

RECOMMENDATIONS:

  1. Hydraulic rig machine or any diesel/petrol driven equipment must be relocated/parked in a safe location after completion of shift activity. No equipment/vehicles shall be kept in unmanned condition in the flare area.
  2. Strict supervision by site supervisors/engineers needs to be implemented for work and also the availability of supervisory staff must be ensured at all workplaces during the changeover of shift.
  3. Arrangement of adequate quantity of fire extinguishers to be ensured at the workplace to meet any exigency.
  4. Regular inspection of equipment to be done by experienced P&M Person.

Brief Description near miss/accident:

After completion of day shift activities, the worker was going to the storeroom for keeping the lifting tools. During this process, he fell down on the ground due to an uneven surface which resulted in his feet twisted.

PROBABLE CAUSE OF THE INCIDENT:

  1. Walking over the uneven ground level.
  2. Poor illumination on the access area.

Lesson learnt / recommendation:

  1. Arrangement of sufficient illumination to be ensured at the workplace including walkways.
  2. Alt access/egress should be leveled and unobstructed.
  3. Be observant of the area around while walking.

Near miss/Accident example – 3

Brief Description near miss/accident:

One loaded Dumper got tilted while it was unloading the excavated earth & clay. The operator could not assess ground conditions before placing the Dumper for unloading which resulted in the tilting of a dumper on one side. No injury noticed.

PROBABLE CAUSE OF THE INCIDENT:

  1. The dumper operator could not position the vehicle properly for unloading the earth/clay due to uneven ground conditions.
  2. Non-availability of experienced Signal Man with the vehicle.

Lesson learnt / recommendation:

  • Availability of experienced Signal Man to be ensured with all heavy equipment/vehicles prior to putting in operation.
  • Duper/Hydra Crane shall be placed on the leveled ground only for operation.

Brief Description near miss/accident:

While shifting the piping spool at elevated location one worker fell down on the same level by hitting with loose kept material.

PROBABLE CAUSE OF THE INCIDENT:

  1. Poor Housekeeping at the workplace.
  2. Heavy construction materials to be shifted mechanically.
  3. Careless behaviors of workers during work.

Lesson learnt / recommendation:

  1. Approach way to be cleared from any obstruction and maintain good housekeeping surrounding the workplace.
  2. Heavy construction materials to be shifted mechanically.
  3. Workers/staff must be attentive at the workplace during works.

Brief Description near miss/accident:

During unloading of pipe from a trailer, one pipe got slipped & fell down on the ground level. No injury/No property damage occurred.

PROBABLE CAUSE OF THE INCIDENT:

  1. Engagement of unskilled workers for unloading the pipes from the trailer.

Lesson learnt / recommendation:

  1. Experienced rigger and supervisor to be engaged for loading & unloading purposes.
  2. Safe materials handling procedure to be adopted by the rigging crew.
  3. Tool Box Meeting must be done prior to allowing the workforces for such activity.

Brief Description near miss/accident:

A crew was engaged at the elevated location for the erection of the pipe, during this process, one loose object freely fell down on the ground level. No property damage /No injury noticed.

PROBABLE CAUSE OF THE INCIDENT:

  1. Inadequate supervision at the workplace.
  2. No provision to arrest the falling objects.
  3. The material was kept in loose condition at an elevated location.

lesson learnt / recommendation:

  1. Loose materials to be secured firmly by suitable means or removed from an elevated location.
  2. Adequate fall protection arrangement to be implemented.

Brief Description near miss/accident:

Whilst a worker was engaged with grinding machine for grinding the Beam, accidentally Grinding Machine slipped from the worker’s grips which resulted grinding machine fell down.

PROBABLE CAUSE OF THE INCIDENT:

  1. Due to inadequate space grinder could not secure the right position to perform the job resulting in the grinding machine slipped during the job.

Lesson learnt / recommendation:

  1. The safe working place to be ensured for Grinder and all workers prior to allowing them to perform any activity.
  2. Experienced Grinder to be allowed at the workplace for grinding jobs.

Brief Description near miss/accident:

During unloading of Gratings from the trailer by using pipes, all of a sudden 4nos of gratings were slipped from the trailer and fell down on the ground. No property damage/No injury observed.

PROBABLE CAUSE OF THE INCIDENT:

  1. The unsafe method adopted to unload the grating from the trailer.

Lesson learnt / recommendation :

  1. Unloading of the grating from Trailer to be done mechanically with an experienced crew. Manually unloading to be avoided strictly.

Near miss/Accident example – 9

Brief Description near miss/accident:

Cable tray support fitting activity @ 11.00mtrs elevation was under progress by the crew. During this process, one cable tray cut piece which was kept close to the work spot fell down on the ground. NO injury noticed.

PROBABLE CAUSE OF THE INCIDENT:

  1. Lack of coordination among workers during works.
  2. The cable tray piece was kept loosely at an elevated location.

Lesson learnt / recommendation:

  1. Proper fall protection arrangement to be ensured.
  2. Effective coordination needs between workers while performing such kind of activity.
  3. Loose materials at an elevated location to be secured firmly by suitable means or removed.
  4. Job-specific training (SAFETY TRAINING) to be provided to the workmen.
