Near-miss/incident case -1

Description of incident:

Structure erection work was under progress at a height of around 20 meters. A tor steel rod (1/2” diameter a & 500mm length) was used for erection work.

After used the tor steel rod, kept on beam; suddenly one worker hand touched/hit the tor steel rod & it fell from 20 meters height.

Root Cause of Incident:

  • Improper securing of loose objects.
  • Restricting the entry of unauthorized personnel into the work zone.
  • Poor supervision.

Preventive action:

  • Barricading the bottom area and keeping a standby person at the barricading area to prevent unauthorized entry into the barricaded zone.
  • Adequate and vigilant supervision.
  • The activity shall be performed after implementing the mitigation measures as per HIRAC.
  • Communication of job procedure along with the risks and its control measures to the workers in the toolbox talk.
  • Doing house-keeping on a daily basis.
  • Securing objects with binding wire/rope to the structure.
  • Safety net shall be tied below the beam double layered(arresting men and material)
  • All loose objects shall be removed from all floors of the technical structure.
  • Metallic bin/jute/Gunny bag shall be used to keep loose materials.

Near-miss/incident case -2

Description of incident:

A worker was moving on the grating floor. Unintentionally he hit the loose pipe piece (pipe length: 1200mm, weight: 2.5kg) and it fell on to ground from 6 meters height.

Root Cause of Incident:

  • After completion of job, the cutting pipes are not removed & the cutting pieces spread the all over elevation;
  • Poor housekeeping.
  • Restricting the entry of unauthorized personnel into the work zone.
  • Poor supervision.

Preventive action:

  • Barricading the bottom area and keeping a standby person at the barricading area to prevent unauthorized entry into the barricaded zone.
  • Adequate and vigilant supervision.
  • The activity shall be performed after implementing the mitigation measures as per HIRAC.
  • Communication of job procedure along with the risks and its control measures to the workers in the toolbox talk.
  • Maintain good house-keeping.
  • Securing objects with binding wire/rope to the structure.
  • Safety net shall be tied below the beam double layered(arresting men and material)

Near-miss/incident case -3

Description of incident:

Dismantling of scaffolding at a first elevation of the structure at a height of 6 meters by one agency scaffolding team (6 workers & 1 engineer) was under progress. All of a sudden one scaffolding pipe, 6 meters length fell from 6 meters height on the worker of other agency, who was moving on the ground floor, it hit the worker on his shoulder and left knee and resulted in swelling. He resumed duties after first-aid.

Root Cause of Incident:

  • Lack of Knowledge.
  • Restricting the entry of unauthorized personnel into the work zone
  • No area barricading.

Preventive action:

  • Coordination with other agencies engineers.
  • Barricading the bottom area and keeping a standby person at the barricading area to prevent unauthorized entry into the barricaded zone.
  • Adequate and vigilant supervision.
  • Adhering to the standard method of dismantling the scaffolding.
  • Securing the pipes while descending to ground level.
  • The activity shall be performed after implementing the mitigation measures as per HIRAC.
  • Communication of job procedure along with the risks and its control measures to the workers in the toolbox talk.

Description of incident:

Grating cutting work by using hand cutting machine at the second elevation of structure around 15 meters height was under progress. All of a sudden one piece of the grating, which he was cutting (weight: 1.2 kg) fell from 15 meters height on the ground.

Root Cause of Incident:

  • The grating piece was not secured.
  • No safety net.

Suggestions/Recommendations:

  • Adequate and vigilant supervision.
  • Barricading the bottom area and keeping a standby person at the barricading area to prevent unauthorized entry into the barricaded zone.
  • Loose materials should be a tie with rope before cutting.
  • Arranging the safety net below the work zone to contain the material before reaching the ground.
  • Communicating in the toolbox talk about the job hazards and the control measures to be adopted while executing the job.

Near-miss/incident case -5

Description of incident:

Chipping of structure around 6 meters height was under progress. All of a sudden workers engaged for chipping activities safety helmet fell from 6 meters height on the ground.

Root Cause of Incident:

  • Wearing helmet without chinstrap.

Suggestions/Recommendations:

  • Wearing the right PPEs in the right way for the right job.
  • Communicating in Tool Box Talk importance of wearing PPEs in the right manner.
  • Height related tool-Box Talk shall be given including PPEs usage.
  • Barricading the bottom area and keeping a standby person at the barricading area to prevent unauthorized entry into the barricaded zone.
